Printed Questionnaire

You can print either the Express or Comprehensive questionnaires. Fill it out and go back and enter responses. 

Here's What You'll Need for Your Screening:

• Your ZIP code

• Your birthday and/or your spouse's birthday

• Where you live: house, apartment or mobile home

• How long have you lived there?

• Are you or your spouse a Veteran?

• Did you or your spouse ever work for the state, local, county or federal government or the railroads?

• What are the names of the medicines you take? (Have your medicine bottles or information with you.)

• How much money do you make in a month?

• What other expenses do you have (such as mortgage/rent, utilities, out-of-pocket medical bills, etc.)?
